The tomb preserves its massive cylindrical drum of travertine blocks with a marble frieze of ox skulls, though the crenellated battlements crowning it were added in the Middle Ages when it was fortified, and its original conical roof is gone. It is administered by Italy's Ministry of Culture as part of the Appian Way Archaeological Park. The tomb and adjoining ruins are open to the public on a ticketed basis.

The Tomb of Cecilia Metella was built on the Via Appia in the late first century BCE as a monumental mausoleum for Caecilia Metella, a Roman noblewoman. In the fourteenth century the Caetani family fortified it into a castle controlling the road. It reflected both late Republican funerary grandeur and medieval control of the Appian Way.
